*Last day to order your jersey for this season is 04/03/2025. If you don't place your order before this date we are unsure of when you will receive your jersey.

*All Jersey sales are final and cannot be canceled or refunded.

*Pick up at AZ Legends Practice when they are ready  

Please fill out the following 

• Jersey Size. PLEASE GO 1 SIZE UP FROM WHAT YOUR CHILD CURRENTLY WEARS IN EVERYDAY SHIRT SIZE* (JERSEYS RUN SMALL)
• Player First and Last Name. Last name will be printed on the jersey. 
• Player Top 3 Jersey Numbers (We will select the jersey number you will receive from those 3)

WE ARE NOT RESPONSIBLE IF YOU ORDER THE INCORRECT SIZE OR SPELL THE LAST NAME WRONG. IF THIS DOES OCCUR AND ITS YOUR FAULT YOU WILL HAVE TO PAY THE JERSEY FEE AGAIN.
